Congratulations on your new employment with Associated Students, Inc. Since 1956, ASI’s primary purpose is to promote and assist in carrying out the educational services of Associated Students, Inc. and to perform other functions related to the student activities of the university.

This web site has been written specifically for you. In it you will find all the information you will need to begin your employment and navigate the onboarding process, which includes:

Attending ASI’s New Hire Orientation, where you will:

  • Learn about ASI’s history and mission.
  • Learn about ASI policies and procedures you will need to follow when on the job
  • Complete required forms with the Human Resources Department


Complete required online training in your first two weeks of working:

  • Ethics in the Workplace Online Training (1 hour)
  • Sexual Harassment (1 hour)
  • General safety training (1.5 hours)

Once you have completed these items, you will be cleared to begin work. HR will be in contact with you and your supervisor throughout this process.

During your first week at ASI, you will be busy learning about the organization, getting your pay and benefits arranged and finding out about your workplace. We want to help you hit the ground running and start your employment as smoothly as possible, so we have organized this page to help answer questions and get you started.

Your Supervisor

Your new supervisor will contact you by phone or e-mail at least one week before your start date. He or she will welcome you and answer any questions you may have. If you do not know who your supervisor is or have not heard from one, simply contact your Human Resource Manager at 562-985-8875.

Day 1 - New Hire Orientation

On your first day of work, you will attend ASI’s New Hire Orientation program.

On your new hire orientation day, you will meet with the ASI Human Resources team to learn about ASI, fill out some paperwork, and attend a short workplace safety presentation.

New full-time staff will learn more about their benefits and have an opportunity to finish completing all necessary forms. In addition, we will begin the identification badge and parking access process.

Identification

Before you start filling out forms, you will need to have a few things ready:

  1. Your student ID card (for new student employees)
  2. A copy of your current class schedule (for new student employees)
  3. Appropriate identification documents to complete the I9 form. Bring one item from LIST A or one item from BOTH LIST B and LIST C (expand table below).

  • All documents must NOT be expired and you must present original documents; no copies or digital images (e.g. phone pictures).
  • LIST A
  • U.S. Passport or U.S. Passport Card
  • Permanent Resident Card or Alien Registration Receipt Card (Form I-551)
  • Foreign passport that contains a temporary I-551 stamp or temporary I-551 printed notation on a machine-readable immigrant visa
  • Employment Authorization Document that contains a photograph (Form I-766)
  • For a nonimmigrant alien authorized to work for a specific employer because of his or her status:
    • Foreign Passport, and
    • Form I-94 or Form I-94A that has the following:
      • The same name as the passport; and
      • An endorsement of the alien’s nonimmigrant status as long as that period of endorsement has not yet expired and the proposed employment is not in conflict with any restrictions or limitations identified on the form.
  • Passport from the Federated States of Micronesia (FSM) or the Republic of the Marshal Islands (RMI) with Form I-94 or Form I-94A indicating nonimmigrant admission under the Compact of Free Association Between the United States and the FSM or RMI

  • LIST B
  • Driver’s license or ID card issued by a State or outlying possession of the United States provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address.
  • ID card issued by federal, state or local government agencies or entities, provided it contains information such as name, date of birth, gender, height, eye color, and address.
  • School ID card with a photograph
  • Voter’s registration card
  • U.S. Military card or draft record
  • Military dependent’s ID card
  • U.S Coast Guard Merchant Mariner Card
  • Native American tribal document
  • Driver’s license issued by a Canadian government authority
  • For persons under age 18 who are unable to present a document listed above:
    • School record or report card
    • Clinic, doctor, or hospital record
    • Day-care or nursery school record.

  • LIST C
  • Social Security Account Number card, unless the card includes one of the following restrictions:
    • NOT VALID FOR EMPLOYMENT
    • VALID FOR WORK ONLY WITH INS AUTHORIZATION
    • VALID FOR WORK ONLY WITH DHS AUTHORIZATION
  • Certification of Birth Abroad issued by the Department of State (Form FS-545)
  • Certification of Report of Birth issued by the Department of State (Form DS-1350)
  • Original or certified copy of birth certificate issued by a State, county, municipal authority, or territory of the United States bearing an official seal
  • Native American tribal document
  • U.S. Citizen ID Card (Form I-197)
  • Identification Card for Use of Resident Citizen in the United States (Form I-179)
  • Employment Authorization document issued by the Department of Homeland Security

As we have mentioned, there are a number of forms you must fill out before you can start work.

The human resources team will guide you through the forms on your new hire orientation day. If you have a question about a form, please contact ASI Human Resources.

  • This form is required to be completed and forwarded to the HR Manager who is arranging your appointment. This form must be signed by immediate supervisor.
  • Personal Action Form (PAF)All employees
  • Purpose
    The form is used to track general information, employee name, effective date of action, and type of action.
    Instructions
    PAF will be distributed at the On-Boarding Session. Employee will complete all applicable areas on form.
  • Federal Employee’s Withholding Allowance Certificate (Form W-4)All employees
  • Purpose
    This form is required by the IRS and is used to determine the correct amount of Federal Income tax to be withheld from you semi-monthly earnings based on the number of exemptions you claim.
    Instructions
    Step 1: Use the Personal Allowances Worksheet to help you determine the number of withholding allowances you are entitled to claim.
    Step 2: Fill out the W4 Form, print one copy, and sign and date form.
  • Employment Eligibility Verification (Form I-9)All employees
  • Purpose
    This form is used to verify your citizenship and eligibility to work in the United States.
    Instructions
    Download and complete Section 1. Print one copy and sign the form. On your first day of work, you will be required to provide sufficient proof of citizenship. Please refer to the back of the form for a list of acceptable documents. The last page of the form provides you with the information on the types of identification you need to bring on your first day of work. Bring one item from Column A, or one item from both Column B and C when you report on your first day.
  • Ethics and Sexual Harassment Awareness TrainingAll employees
  • Purpose
    This form is used as reminder for the two on-line courses that must be completed within two weeks after your first day of work.
    Instructions
    Read, complete, print and sign.
  • Predesignation of Personal Physician (Optional)All employees
  • Purpose
    In the event you sustain and injury or illness related to your employment, you may be treated for such injury or illness by your personal medical doctor (M.D.), doctor of osteopathic medicine (D.O), or medical group.
    Instructions
    Read, complete, print and sign the form.
  • Mobile Service Devices Usage AcknowledgementAll employees
  • Purpose
    To inform employees of the California Wireless Telephone Automobile Safety Act prohibiting California drivers from using handheld wireless telephones while driving unless the devices are configured to allow hands-free listening and talking.
    Instructions
    Read, complete, print and sign the form.
  • Meal and Rest Period Obligation Acknowledgment Non-Exempt Salaried and Hourly EmployeesAll employees
  • Purpose
    To inform employees of the California Wage Order that requires that all employees must take a meal period of no less that 30 minutes whenever time exceed five hours in a work day and must begin before exceeding five hours of work. Employees are entitled to a rest period of ten (10) minutes for 3.5 to 6 hours of work.
    Instructions
    Read, complete, print and sign the form.
  • Direct Deposit Sign-Up Form (Optional)All employees
  • Purpose
    This form is used to identify the financial institution where your semi-monthly earnings will be forwarded through electronic funds transfer.
    Instructions
    Read, complete, print and sign. Submit a copy of a voided check along with your form to avoid errors.
  • IIPP Form A All employees
  • Purpose
    The purpose of this form is to acknowledge that you have been trained on various aspects of ASI’s Illness and Injury Prevention Plan (IIPP).
    Instructions
    Click on the link, and enter your name, your campus email address, and your supervisors name and email address. Enter your name and ID number, check the boxes required to acknowledge you have been trained on the respective topics, sign, and submit.
  • IIPP Form B All employees
  • Purpose
    This form is used to confirm that you have access to ASI’s code of safety practices (pages 15-18 of the IIPP).
    Instructions
    Enter your full name and ID number, sign, and submit.
  • Foreign National Information Foreign Employees
  • Purpose
    This form is required to verify your immigration status.
    Instructions
    A copy of both sides of your I-94, your US Visa from your passport, the picture page of your passport and an I-20 or DS2019 form must be submitted with this form. Complete all applicable information as determined by your immigration status. Print both sides of the form, sign and date it.
  • New Hire Checklist and AcknowledgementsAll employees
  • Purpose
    To help employees acknowledge and collect required forms. The following training material will inform employees of workplace safety and health, company-wide policies and procedures, and work environment policies.
